Glucose Management Team

The inpatient Glucose Management Team (GMT) at the University of 麻豆传媒高清 Hospital has grown by leaps and bounds since it was started in 2008 by Dr. Boris Draznin. The team includes the following members:

(Director)
, DNP, MS, RN
, PA-C, PA-C (APP Co-Lead)
, PA-C, NP, MSN (APP Co-Lead)
, PA-C, CNS, NP, MS
, PA-C, PA-C
, PA-C, MPAS, DNP, MSN
Eileen Findlay, PA-C, NP, MS, BSN
The mission of the UCHealth Glucose Management Team is to work with patients, clinical teams, nursing and other staff, to achieve good glycemic control and to plan for hospital discharge. Poor glycemic control in the hospital setting is associated with worse clinical outcomes, but management of diabetes and stress hyperglycemia without causing hypoglycemia is complex. There are many changing factors and the need for close monitoring and adjustments.
The Glucose Management Team works with providers and teams across University Hospital. The team is an integral part of maintaining Joint Commission on Accreditation of Healthcare Certificate of Distinction for Inpatient Diabetes Care for UCHealth Anschutz, and maintaining Top Performer status in the Society of Hospital Medicine eQuips Glucometrics Benchmarking Program.
In addition to providing direct patient care through consults and follow-up visits in the hospital, the Glucose Management Team educates fellows, housestaff, and advanced practice fellows and students in inpatient glycemic management, and is involved in clinical research and quality improvement initiatives.
