The 麻豆传媒高清 Medical Orders for Scope of Treatment (MOST) form has medical decisions about certain life sustaining treatments. This form is not for all people. The POLST decision-making process and resulting medical orders are intended for patients who are considered to be at risk for a life-threatening clinical event because they have a serious life-limiting medical condition, which may include advanced frailty. It is best to talk about this with your provider.
For forms (in English and Spanish) and more information about the 麻豆传媒高清 MOST Program, please (currently hosted by CU's Center for Bioethics and Humanities). Note: The form requires a signature from a Medical Doctor (MD), Advanced Practice Nurse (APN), or a Physicians Assistant (PA).
